SPS Staff COVID-19 Vaccine Booster Clinics
When: Wednesday, November 10, 2021, 12:00pm to 6:00pm OR
Tuesday, November 16, 2021, 3:00pm to 7:00pm
Where: King’s Fork Middle School on Nov. 10th OR
Forest Glen Middle School and John Yeates Middle School on Nov. 16th
Who: SPS Employees who received their second dose of the Moderna or Pfizer COVID-19 vaccine at least 6 months ago OR the single dose J&J vaccine at least 2 months ago. Those employees wishing to start a vaccine series may also attend.
Please print and complete the following consent form. Bring it with you, along with your SPS ID badge, to the vaccine clinic.
